Hood vent system inadequately collecting grease and condensation. Ammonium compound solution failed to meet parameters to achieve sanitation. Saw food employee exit the sushi bar area and went to dining room to check phone, employee then returned to sushi area and reused gloves without washing hands. Saw cut lemons in the handwashing sink. Saw pests in the warewashing machine, employee office and shelves near sushi. There were substantial rodent droppings and one dead cockroach. Instructed person in charge to provide Environmental Health Department with standard operating procedures and pest control plan. Instructed person in charge to have a professional pest control company set traps and perform treatment as needed. Oberserved warewashing machine with buildup of rodent droppings. Debris buildup on walls. Observed frozen food improperly thawing. Seafood was thawing on prep table. Sanitizer buckets stored on the ground.

Result: Immediate closure
